Hot asphalt burns: a review of injuries and management options

摘要:Hot asphalt burns to human tissue can increase the likelihood of infection and potential conversion of partial thickness to full-thickness injuries. Successful intervention for hot asphalt burns requires immediate and effective cooling of the asphalt on the tissue followed by subsequent gradual removal of the cooled asphalt. A review of the literature reveals that multiple substances have been used to remove asphalt, including topical antibiotics, petroleum jelly, a commercial product known as De-Solv-It (ORANGE-SOL, Chandler, AZ), sunflower oil, baby oil, liquid paraffin, butter, mayonnaise, and moist-exposed burn ointment (MEBO). Although many of these products may be effective in the removal of asphalt, they may not be readily available in an emergency department setting. Topical antibiotics are readily available, are more commonly described in the medical literature, and would be expected to be effective in the removal of asphalt. We developed guidelines for on scene (first-aid) management and the initial care of such patients upon presentation to a health care facility. These guidelines emphasize the principles of early cooling, gradual removal of adherent asphalt using topical antibiotics, and avoidance of the use of topical agents, which are likely to result in tissue toxicity.

External application of moisture exposed burn ointment for phlebitis: A meta-analysis of randomized controlled trials

摘要:To evaluate the therapeutic effects of moisture exposed burn ointment (MEBO) on phlebitis, seven electronic databases where checked until September, 2016 for randomized controlled trials (RCTs) of MEBO on phlebitis. Risk of bias was assessed using Cochrane handbook guidelines. Thirty eight randomized controlled trials met the inclusion criteria in which the aggregated results indicated that comparison revealed significant differences in total effectiveness rate of MEBO versus conventional therapy (RR=1.27, 95% confidence interval [CI]=1.06, 1.52, and P=0.009), and there were some beneficial evidence regarding the effects on reducing incidence of phlebitis MEBO versus conventional therapy in preventing phlebitis (RR=2.73, 95% confidence interval [CI]=1.94, 3.85, and P<0.00001). The evidence that MEBO is an effective treatment for phlebitis is encouraging, but not conclusive due to the low methodological quality of the RCTs. Therefore, more high-quality RCTs with larger sample sizes are required.   Key words: External application of moisture exposed burn ointment, prevention and (or) treatment, phlebitis.

Expression and predictive value of HIF-1α and VEGF in patients with burns following treatment

摘要:The present study aimed to investigate the expression and predictive value of serum hypoxia-inducible factor-1α (HIF-1α) and vascular endothelial growth factor (VEGF) in patients with burns following treatment. A total of 84 patients with burns treated in Jinan City People's Hospital (Jinan, China) between June 2015 and August 2017 were selected and their clinical information was collected. The expression levels of HIF-1α and VEGF before and after treatment were detected via ELISA, and HIF-1α and VEGF levels in patients with effective and ineffective treatment were compared. The predictive values of HIF-1α and VEGF in clinical efficacy were determined using receiver operating characteristic (ROC) curves, and independent risk factors affecting treatment inefficacy were analyzed via multivariate logistic regression. It was revealed that HIF-1α decreased significantly (P<0.05) while VEGF significantly increased in patients after treatment. Patients with effective treatment presented significantly lower HIF-1α levels and higher VEGF levels compared with those with ineffective treatment. The ROC curve indicated that the area under the curve (AUC) of HIF-1α for treatment efficacy was 0.795, the 95% CI was 0.666-0.924, the specificity and sensitivity were 68.75 and 80.88%, respectively, and the Youden index was 49.63%. For VEGF, the AUC, 95% CI, specificity, sensitivity and Youden index were 0.826, 0.725-0.928, 68.75, 82.35 and 51.10% respectively. Moreover, under the joint detection of HIF-1α and VEGF, the AUC was 0.847, 95% CI was 0.746-0.947, specificity and sensitivity were 87.50 and 66.18%, respectively, with a Youden index of 53.68%. Multivariate analysis demonstrated that higher HIF-1α level, lower VEGF level and higher burn degree before treatment were independent risk factors for treatment inefficacy. HIF-1α levels decreased and VEGF levels increased in burn patients after treatment. HIF-1α and VEGF before treatment may therefore serve as predictors for treatment efficacy.

摘要:Objective: To verify the moisture maintaining effect of moist exposed bum ointment (MEBO) in treating burn wound. Methods: Rabbit model with Ⅱdegree burn wound was used in this study. Observations were conducted at 0 (before burn), 0. 5, 6, 24, 48, 72, and 168 hours postburn. The data of wound water evaporation, wound appearance and pathological changes of the wounds were compared with that Of the bum wounds treated with Vaseline or conventional dry exposed therapy. Results: It was found that wounds treated with MEBO had water evaporation close to that of normal skin and was significantly less than that of the wounds treated with dry exposed therapy (P<0. 01). MEBO did not obstruct water evaporation from wound as Vaseline did (P<0.01). Wounds treated with MEBO had good permeability and adequate drainage. Conclusion: MEBO provided the wounds with a physiological moist environment.
