The Antibacterial Effect of Phytosterols Isolated from Echinops heterophyllus in Comparison with MEBO? and Standard Antimicrobial Agents.

摘要:This study investigated the types of phytosterols that exists in the aerial and root parts of Echinops heterophyllus and explored the antibacterial effect of these phytosterols against some pathogenic bacteria in comparison with Moist Exposed Wound Ointment (MEBO?) and gentamicin.Plant was extracted and phytosterols were isolated and purified from the crude extract of Echinops heterophyllus by using chemical method. Thin Layer Chromatography (TLC) detected the presence of steroids in the aerial and roots part and gas chromatographymass spectrophotometer GC/MS analysis was used to identify some of these compounds and among them were beta-sitosterol and stigmasterol. Standard dilutions of the phytosterols isolated from Echinops heterophyllus were made from 10-100%; its antibacterial effect had been examined by seeded agar method against Staphylococcus aureus and Pseudomonas aeruginosa.The effect of these phytosetrols against Staphylococcus aureus was more potent than against Pseudomonas aeruginosa.The minimum inhibitory concentration (MIC) was seventy mg/ml and the minimum bactericidal concentration (MBC) was eighty five mg/ml. This study illustrates that sitosterol isolated from Echinops heterophyllus has a greater inhibitory effect on the test bacteria than gentamicine and closer results to MEBO?. It can be concluded in this study is a good step to show the types of phytosterols isolated from Echinops heterophyllus and the antibacterial effectof these phytosterols in vitro against Staphylococcus aureus and Pseudomonas aeruginosa.

Successful Combined Treatment with Total Parenteral Nutrition Fluid Extravasation Injuries in Preterm Infants

摘要:Extravasation injuries in the neonatal intensive care unit are not rare during parenteral hyperalimentation. There have been many different methods of management. We report five premature infants with wounds of hyperalimentation fluid extravasation managed by the antibacterial ointment (Terramycin ophthalmic ointment?) and sesame oil and a antiinflammatory herbal mixture (MEBO?). The mean gestational age of patients was 31+2 weeks (range, 28+4 to 35+6 weeks), and the mean weight at extravasation was 1,930 g (range, 1,140 to 2,680 g). Extravasation occurred within the mean of 32 days (range, 17 to 50 days). The method of dressing was application of a thick layer of this mixture covered by vaseline and wet gauze renewed at an interval of 8-12 hr after irrigating the wounds thoroughly with normal saline. The mean duration of dressing was 30 days (range, 20-50 days). The wounds had healed completely leaving a small size of contracture without functional abnormality. We conclude that this therapy may be considered for an alternative treatment and warrants clinical trials for the confirmation of the local management of extravasation injury.

摘要:Purpose:? Acute radiation dermatitis is a common side-effect of radiotherapy in breast cancer and has a profound impact on patients’ quality of life, due to pain and discomfort. The aim of this study is to compare the effect of β-sitosterol (Mebo) ointment to trolamine (Biafine) cream for the prevention and treatment of radiation dermatitis in breast cancer patients receiving adjuvant radiation therapy. Materials and Methods:? This is a prospective open-label randomized phase III study developed to assess the efficacy of 2 topical agents used for management of acute radiation dermatitis. Female breast cancer patients who needed a course of radiation therapy in our institution were enrolled and randomized into 2 groups 1 with Mebo ointment and 1 with Biafine cream. Both medications were applied twice per day during the whole period of treatment and skin reactions and related symptoms were assessed weekly during the entire course. Grading of skin reactions was done according to the Radiation Therapy Oncology Group grading system. Results:? Between September 2015 and May 2017, a total of 161 patients were recruited for this trial. Mean age was similar for both groups (50.19±12.57 vs. 51.73±11.23, respectively, P=0.41). All other patients and treatment characteristics were similar in both groups, except for the use of boost (82.7% in the Biafine group vs. 36.7% in Mebo group, P=0.012). Analysis was done for reactions recorded before the beginning of the boost and for the entire course including the boost. Using univariate and multivariate analysis, there was no significant difference in grades 2 and 3 dermatitis between the 2 groups. However, the incidence of severe pruritus and severe local skin pain were both significantly reduced in the Mebo group (14.1% in Biafine vs. 2.9% in Mebo, P=0.016 for pruritus and 11.5% vs. 1.4%, respectively, P=0.02 for severe pain). Conclusions:? This study showed no difference between Mebo and Biafine in the incidence and severity of breast skin dermatitis during radiation therapy. However, the use of Mebo ointment was associated with decreased severe pruritus and pain which could positively affect patient comfort and quality of life.
