摘要:CONTEXT: Moist exposed burn ointment (MEBO), from China, has been said to revolutionize burn management. OBJECTIVE: Our study was conducted to compare MEBO with conventional management (C) with respect to the rate of wound healing, antibacterial and analgesic effect, and hospital costs. DESIGN: This is a prospective, randomized, controlled clinical trial conducted between 1 March 1997 and 24 October 1998. SETTING: The trial was conducted in a specialized burn facility located in a tertiary referral hospital in a developed and industrialized island-state in Southeast Asia. PATIENTS: We randomly assigned 115 consecutive patients between the ages of 12 and 80 who had partial-thickness thermal burns covering less than 40% of body surface area (BSA) to receive either MEBO or C. Fifty-seven patients were assigned to MEBO and 58 patients to C. The latter group received twice-daily dressing changes; MEBO patients received MEBO every 4 hours. MAIN OUTCOME MEASURES: Patients were hospitalized until 75% BSA had healed. BSA was determined by visual inspection and charted on Lund and Browder charts regularly. Wound healing rate, bacterial infection rate, pain score, and hospitalization costs were recorded. RESULTS: The median time to 75% healing was 17.0 and 20.0 days with MEBO and C, respectively (HR = 0.67, 95% CI = 0.41-1.11, P =.11), suggesting similar efficacy between the 2 modalities. Bacterial infection rates were similar between the 2 groups (HR = 1.10, 95% CI = 0.59-2.03, P =.76). MEBO imparted a greater analgesic effect in the first 5 days of therapy and reduced hospital costs by 8%. CONCLUSIONS: MEBO is as effective as conventional management but is not the panacea for all burn wounds. The use of MEBO eases the management of face and neck burns and facilitates early institution of occupational therapy in hand burns. It confers better pain relief such that fewer opiates are used during the first 5 days after burn injury.

摘要:Background: Despite the availability of many topical formulations for scar treatment, an objective evaluation of their effectiveness is lacking. Objective: To assess the physiological, structural, and mechanical parameters of scar healing by a scar ointment (MEBO-Scar). Methods: 45 patients from the Department of Obstetrics at Al Ain Hospital, UAE, with recently acquired Pfannenstiel scars were recruited into this prospective, open-label, controlled study. Scar ointment was applied to a lateral third of the scar; positive control (silicone gel based topical) to the other lateral third; the middle third was left untreated (negative control). The scars were evaluated at 4 time points up to 12 weeks using non-invasive devices and a scar questionnaire. Results: In comparison with silicone gel, there was a significant reduction in trans epithelial water loss at weeks 2, 4, 8, and 12 (p<0.001) and scar area at weeks 4, 8 and 12 (p<0.001) with scar ointment. The skin elasticity at week 4 was significantly increased by scar ointment (p=0.007). By week 12, gross colour mismatch of the scar was described by 9% patients with scar ointment (24% with silicone gel, 42% with no treatment; p<0.001). There was a significant improvement in scar texture with scar ointment by week 12 (p<0.001). Conclusion: Scar ointment has beneficial effects on objective and subjective patient-reported parameters of Pfannenstiel scars.

摘要:Background MEBO is an analgesic and antibacterial natural agent. Sesame oil extract plays a positive effect in wound healing that may be attributed to its ability to scavenge free radicals and antioxidant properties. Material and methods In this study, MEBO was topically applied to the site of a traumatic oral ulcer to examine its therapeutic effect on pain relief and wound healing. Patients were randomly assigned into two equal groups: group I (MEBO), group II (control), (n?=?15 patients each). Pain was recorded by using the Visual Analogue Scale (VAS), wound size was measured at days 2, 5, and 10. Cytological analysis was also performed using the superficial epithelial cell and keratinization indices. Results There was a statistically significant difference in VAS scores and wound size between MEBO group and control group (p?≤?0.001). Cytological results of MEBO group showed a highly significant improvement (p?≤?0.00001) in re-epithelization and keratinization indices over time. Conclusion Our results showed that the application of MEBO on traumatic ulcer sites has a significant improvement in wound healing and rapid enhancing effect on the re-epithelialization rate.

摘要:AbstrakBerbagai penelitian ilmiah telah membuktikan bahwa kolagen merupakan biomaterial ideal dalam aktivitas penyembuhan luka. Senyawa bioaktif pada madu dan Aloe vera dapat membantu meningkatkan pembentukan kolagen. Penelitian ini dilakukan untuk membandingkan pengaruh aplikasi topikal madu, gel Aloe vera, dan Moist Exposed Burn Ointment (MEBO) terhadap kepadatan kolagen pada proses penyembuhan luka bakar derajat dua pada tikus. Luka bakar dibuat pada 28 ekor tikus dan dibagi menjadi empat kelompok yang dipilih secara acak serta dilakukan aplikasi topikal harian dengan NaCl, madu, gel Aloe vera, dan MEBO secara berurutan. Biopsi kulit dilakukan pada hari ke-7, kemudian dilakukan pembuatan sediaan histopatologi kulit dan dihitung kepadatan kolagennya. Uji One Way ANOVA menunjukkan kepadatan kolagen berbeda secara signifikan antar masing-masing kelompok (p= 0,009). Uji Post Hoc Bonferroni menunjukkan terdapat perbedaan yang signifikan antara kelompok NaCl dengan madu (p= 0,024) dan NaCl dengan MEBO (p= 0,024). Penelitian ini menunjukkan bahwa aplikasi madu dan MEBO secara topikal pada luka bakar derajat dua dapat meningkatkan pembentukan kolagen, sehingga dapat mempercepat proses penyembuhan luka. The bioactive compounds in honey and Aloe vera can help increase collagen formation. This study was conducted to compare the effect of topical application of honey, Aloe vera gel, and MEBO (Moist Exposed Burn Ointment) on collagen density in the healing process of second-degree burns in rats. Burns were made on 28 rats which were further randomly divided into four groups to receive daily topical application of NaCl, Aloe vera, honey, and MEBO respectively. Skin biopsy was carried out on the seventh day, then histopathological preparation of the skin was made and collagen density was calculated. Result of One Way ANOVA test showed that collagen density differed significantly between groups (p= 0.009). The Post Hoc Bonferroni test resulted in significant difference between NaCl with honey group (p= 0.024) and NaCl with MEBO (p= 0.024). This study found that topical application of honey and MEBO to second-degree burns could increase collagen formation, thus accelerating wound healing process. Honey, Aloe vera, and MEBO can be used as alternative therapies for healing burns.
