The signaling pathways of traditional Chinese medicine in promoting diabetic wound healing

摘要:Ethnopharmacological relevance The diabetic wound is one of the common chronic complications of diabetes, which seriously affects patients’ quality of life and even causes disability and death. Traditional Chinese medicine (TCM) is a unique and precious resource in China, which has a good curative effect and safety. At present, it has been found that Chinese herbal compounds and effective active ingredients can effectively promote diabetic wound healing, and its mechanism needs to be further studied. Signaling pathways are involved in the pathogenesis and progression of diabetic wounds, which is one of the main targets for the pathologic mechanism of diabetic wounds and the pharmacological research of therapeutic drugs. Aim of the review This study has been carried out to reveal the classical signaling pathways and potential targets by the action of TCM on diabetic wound healing and provides evidence for its clinical efficacy. Materials and methods “diabetic wound”, “diabetic foot ulcer”, “traditional Chinese medicine”, “natural plant” and “medicinal plant”, were selected as the main keywords, and various online search engines, such as PubMed, Web of Science, CNKI and other publication resources, were used for searching literature. Results The results showed that TCM could regulate the signaling pathways to promote diabetic wound healing, such as Wnt, Nrf2/ARE, MAPK, PI3K/Akt, NF-κB, Notch, TGF-β/Smad, HIF-1α/VEGF, which maintaining inflammatory interaction balance, inhibiting oxidative stress and regulating abnormal glucose metabolism. Conclusion The effect of TCM on diabetic wound healing was reflected in multiple levels and multiple pathways. It is envisaged to carry out further research from precision-targeted therapy, provide ideas for screening the core target of TCM in treating diabetic wounds and create modern innovative drugs based on this target.

MEBT/MEBO对慢性创面相关miRNA-203的调节效应

摘要:目的:许多miRNAs对皮肤有特定功能,miRNA广泛参与了机体皮肤发育,包括表皮细胞的增殖、分化、衰老及毛囊的发育,miRNA-203是第一个被发现在表皮及毛囊丰富表达的miRNA, miRNA-203在皮肤中的表达比在其他器官中高出100倍,被称为是"皮肤特异性的miRNA",它可通过直接抑制p63基因表达而促进表皮分化。研究表明,miRNA-203表达与皮肤分化和分层密切相关。临床实践中应MEBT/MEBO治疗慢性难愈合性创面,发现MEBO能显著缩短创面愈合时间,减少瘢痕愈合;进一步研究发现MEBO/MEBO能明显提高创面肉芽组织VEGF、bFGF、EGF基因及蛋白表达,最终加快创面愈合的进程,减少疤痕形成。尽管我们的研究初步揭示了MEBO防治慢性难愈合创面的部分机制,但是MEBO是通过何种途径调节以上基因及蛋白表达的呢?这个问题还有待进行更深入的研究。鉴于miRNA是一类小分子的非编码调控基因,而且人类中大部分的蛋白编码基因受miRNA调控,所以我们研究目的是证明MEBT/MEBO可以通过对miRNA-203的调控来促进慢性难愈合创面恢复。方法:参照付小兵等全层皮肤缺损法和沈氏改良塑料环肉芽肿定量法经改进制成大鼠慢性难愈合创面模型。健康成年SPF级Wistar雄性大鼠250只,12周龄,体重220g~250g,适应性喂养1周后,随机分为:空白组50只、对照组50只、慢性难愈合创面组150只。空白组大鼠不作任何有损伤性处理;对照组大鼠按上述模型建立方法在造模部位行全层皮肤切除,但不注射氢化可的松;慢性难愈合创面组大鼠按上述方法制备慢性难愈合创面模型,然后按处理方法随机分为模型组、MEBO组和贝复济组,每组50只,各组创面制备后立即用药治疗。模型制备完成后,各组于1d、3d、7d、11d、14d共5个时相点随机取10只大鼠,尾静脉注射适量的多功能纳米探针,然后利用小动物活体成像系统,进行miRNA-203的活体原位荧光成像与分析检测。活体原位荧光成像与分析检测完成后,对大鼠作病理切片,Western Blot检测HIF-1α、PTEN、VEGFR2、PI3K、MMP2、p-Akt蛋白表达。结果:MEBO可以通过PTEN/Akt/VEGF信号通路影响血管内皮细胞的生长,调控血管新生;通过PTEN/Akt/EGF信号通路影响表皮细胞的增殖和爬行,调控再上皮化的过程,进而促进细胞生长因子产生和修复创面。而MEBO可以促进miRNA-203的表达,而miRNA-203又可以调节PTEN表达水平的变化。结论:PTEN-Akt-VEGF/EGF通路可以接受miRNAs调节而发挥相应作用,而MEBT/MEBO可以通过对miRNA-203的调控来促进慢性难愈合创面恢复。所以miRNA-203在MEBO发挥防治慢性难愈合创面作用过程中起到重要的作用。

Expression and predictive value of HIF-1α and VEGF in patients with burns following treatment

摘要:The present study aimed to investigate the expression and predictive value of serum hypoxia-inducible factor-1α (HIF-1α) and vascular endothelial growth factor (VEGF) in patients with burns following treatment. A total of 84 patients with burns treated in Jinan City People's Hospital (Jinan, China) between June 2015 and August 2017 were selected and their clinical information was collected. The expression levels of HIF-1α and VEGF before and after treatment were detected via ELISA, and HIF-1α and VEGF levels in patients with effective and ineffective treatment were compared. The predictive values of HIF-1α and VEGF in clinical efficacy were determined using receiver operating characteristic (ROC) curves, and independent risk factors affecting treatment inefficacy were analyzed via multivariate logistic regression. It was revealed that HIF-1α decreased significantly (P<0.05) while VEGF significantly increased in patients after treatment. Patients with effective treatment presented significantly lower HIF-1α levels and higher VEGF levels compared with those with ineffective treatment. The ROC curve indicated that the area under the curve (AUC) of HIF-1α for treatment efficacy was 0.795, the 95% CI was 0.666-0.924, the specificity and sensitivity were 68.75 and 80.88%, respectively, and the Youden index was 49.63%. For VEGF, the AUC, 95% CI, specificity, sensitivity and Youden index were 0.826, 0.725-0.928, 68.75, 82.35 and 51.10% respectively. Moreover, under the joint detection of HIF-1α and VEGF, the AUC was 0.847, 95% CI was 0.746-0.947, specificity and sensitivity were 87.50 and 66.18%, respectively, with a Youden index of 53.68%. Multivariate analysis demonstrated that higher HIF-1α level, lower VEGF level and higher burn degree before treatment were independent risk factors for treatment inefficacy. HIF-1α levels decreased and VEGF levels increased in burn patients after treatment. HIF-1α and VEGF before treatment may therefore serve as predictors for treatment efficacy.
