MEBT/MEBO调控肌成纤维细胞促进糖尿病难愈合创面修复的机制研究

摘要:目的:探讨湿润暴露疗法/湿润烧伤膏(Moist exposed burn therapy/moist exposed burn ointment,MEBT/MEBO)对糖尿病难愈合创面的作用,并以α-平滑肌肌动蛋白(α-smooth muscle actin,α-SMA)的表达水平反映糖尿病难愈合创面肌成纤维细胞转化程度,影响细胞外基质主要成分Ⅰ型胶原蛋白(Collagen I,COL-1)的生成为切入点,通过动物及细胞实验探讨MEBT/MEBO促进糖尿病难愈合创面的具体机制,为MEBT/MEBO的临床应用及推广提供理论依据。方法:(1)90只SPF级成年雄性Wistar大鼠以随机数表法分为五组,空白组、对照组、模型组、MEBO组、贝复新组,每组18只,适应性喂养7天后应用STZ腹腔注射法诱导糖尿病大鼠模型,成模后再以全层皮肤缺损法统一制备创面。观察创面愈合情况,拍照记录创面形成后第0、3、7、14天的大鼠创面,并随机切取每组6只大鼠创面肉芽组织,进行HE、Masson染色以观察各组创面组织病理形态变化,免疫荧光观察α-SMA、COL-1的分布及表达情况,Western blot检测α-SMA、COL-1的蛋白表达水平。(2)探讨MEBT/MEBO对大鼠真皮成纤维细胞(rat dermal fibroblasts,RDFs)的作用。首先用双酶法(中性蛋白酶+胰酶)分离提取Wistar大鼠幼崽皮肤真皮成纤维细胞,以HE染色观察细胞形态,免疫荧光法鉴定其波形蛋白(Vimentin)表达情况。以CCK8法摸索MEBO对RDFs的合适剂量,细胞实验分为低糖组、高糖组、溶剂组、低剂量MEBO组、高剂量MEBO组,DCFH-DA荧光探针法检测各组活性氧(reactive oxygen species,ROS)水平,实时定量PCR法检测α-SMA、COL-1的基因转录水平,Western blot法检测RDFs的α-SMA、COL-1蛋白表达水平。结果:(1)大鼠经STZ法诱导糖尿病模型后,出现典型“三多一少”症状,血糖明显升高达到糖尿病标准。与其他组相比,模型组大鼠出现创面愈合延迟(P<0.05),创面肉芽组织持续炎症浸润、血管新生不足、成纤维细胞数量低下及细胞外基质沉积减少等特征;治疗中后期创面愈合率:对照组(29)MEBO组=贝复新组(29)模型组。(2)WB:与对照组比较,创面发生第7、14天过程中,模型组创面组织α-...

MEBT/MEBO再生医疗技术治疗臀部肿物切除推进皮瓣成形术后皮瓣坏死1例临床应用

摘要:目的观察MEBT/MEBO再生医疗技术治疗手术后皮瓣坏死(arbitrary flapnecrosis)的临床疗效。方法对2018年1月宜宾市工人医院外二科收治的1例臀部肿物切除推进皮瓣术后皮瓣坏死患者予以创面MEBT/MEBO再生医疗技术换药,观察治疗效果。结果治疗94 d后,患者创面基本愈合,出院后继续门诊换药1月创面完全愈合,无明显瘢痕增生。结论 MEBO湿润烧伤膏治疗推进皮瓣坏死,可有效促进创面的愈合,疗效显著,且操作简便,值得临床推广应用。

摘要:Background: Burns are considered a serious health affection that leads to several consequences affecting a person both physically and emotionally. Herbal and traditional medicine have become popular remedies among patients worldwide. Aim: This study explores common practices followed in burns as first aid management. Methods: A cross-sectional study was conducted in the Eastern province of Saudi Arabia using a designed questionnaire distributed electronically through different social media. The questionnaire consisted of socio-demographic details, history of burns, causative material, and practices followed in response to burns. Results: 461 individuals have participated in this study. The commonest reason for burns was hot water or steam. The majority of the respondents (63%) had a satisfactory response to self-aid alone at home. The most common first aid options for managing burns at home were cold water alone 195 (42%), any sort of cream alone 177 (38%), or both 317 (69%). Overall, the result shows no statistically significant difference between the outcome of burn injury and the most commonly used burn aids. Conclusions: Most people use creams and water as the first-aid management of burns, while a good number of people use traditional medicine. Overall, people who receive hospital treatment after getting first aid at home give a better outcome.

摘要:Following traumatic or surgical injury to the skin, wounds do not heal by tissue regeneration but rather by scar formation. Though healing is definitely a welcomed event, the resultant scar, very often, is not aesthetically pleasing, and not infrequently, may be pathologic causing serious deformities and contractures. Management of problematic scars continues to be a frustrating endeavor with less than optimal results. Prophylactic methods of wound management to minimize serious scarring are being developed. In a previously published study, we have demonstrated improved healing of split thickness skin graft donor sites following treatment with Moist Exposed Burn Ointment (MEBO, Julphar Gulf Pharmaceutical Industries, Ras Al-Khaimah, UAE). At present, we are reporting the results of a comparative clinical prospective study evaluating scar quality following primary healing of elective surgical and traumatic facial wounds with prophylactic MEBO application, topical antibiotic ointment application, and no topical therapy at all. Scars were evaluated according to the Visual Analogue Scale for scar assessment. Statistical analysis of scar assessment scores demonstrated marked prevention of unfavorable scars with improved cosmetic results following MEBO prophylactic therapy.
