Improved Scar Quality Following Primary and Secondary Healing of Cutaneous Wounds

摘要:Poor wound healing remains a critical problem in our daily practice of surgery, exerting a heavy toll on our patients as well as on the health care system. In susceptible individuals, scars can become raised, reddish, and rigid, may cause itching and pain, and might even lead to serious cosmetic and functional problems. Hypertrophic scars do not occur spontaneously in animals, which explains the lack of experimental models for the study of pathologic scar modulation. We present the results of three clinical comparative prospective studies that we have conducted. In the first study, secondary healing and cosmetic appearance following healing of partial thickness skin graft donor sites under dry (semi-open Sofra-Tulle dressing) and moist (moist exposed burn ointment, MEBO) was assessed. In the second study, healing of the donor sites was evaluated following treatment with Tegaderm or MEBO, two different types of moisture-retentive dressings. In the third study, 3 comparable groups of primarily healed wounds were evaluated. One group was treated by topical antibiotic ointment, the second group was treated by Moist Exposed Burn Ointment (MEBO), and the third group did not receive any topical treatment. In the second study, secondary healing of partial thickness skin graft donor sites was evaluated following treatment with Tegaderm or MEBO, two different types of moisture-retentive dressings. In the second and third studies, healed wounds were evaluated with the quantitative scale for scar assessment described by Beausang et al. [1] Statistical analysis revealed that for both types of wound healing, scar quality was significantly superior in those wounds treated with MEBO.

摘要:The present study aimed to compare between the use of platelet rich fibrin membrane (PRFM) and MEBO scar ointment for cutaneous wound healing as a model treatment. For this reason, forty two local breed rabbits have been divided into four groups, the first three groups were subjected to full thickness skin wound using biopsy punch instrument (diameter 8 mm) on the dorsal thoracic side. Rabbits in the first group leaved without treatment, while in the second group we planted the PRFM in biopsy site, the third group treated with MEBO scarointment. Wound healing process was observed grossly and microscopically at day 3, 7, 10 and 12 post operation.All groups showed normal healing process with competitive advantage to PRFM and MEBO groups respectively compared with untreated group, PRFM was better than the MEBO group, the advantage characterized by less inflammatory reaction, fast reepitheliazation, granulation tissue formation, fibroplasias, and angiogenesis.

摘要:CONTEXT: Moist exposed burn ointment (MEBO), from China, has been said to revolutionize burn management. OBJECTIVE: Our study was conducted to compare MEBO with conventional management (C) with respect to the rate of wound healing, antibacterial and analgesic effect, and hospital costs. DESIGN: This is a prospective, randomized, controlled clinical trial conducted between 1 March 1997 and 24 October 1998. SETTING: The trial was conducted in a specialized burn facility located in a tertiary referral hospital in a developed and industrialized island-state in Southeast Asia. PATIENTS: We randomly assigned 115 consecutive patients between the ages of 12 and 80 who had partial-thickness thermal burns covering less than 40% of body surface area (BSA) to receive either MEBO or C. Fifty-seven patients were assigned to MEBO and 58 patients to C. The latter group received twice-daily dressing changes; MEBO patients received MEBO every 4 hours. MAIN OUTCOME MEASURES: Patients were hospitalized until 75% BSA had healed. BSA was determined by visual inspection and charted on Lund and Browder charts regularly. Wound healing rate, bacterial infection rate, pain score, and hospitalization costs were recorded. RESULTS: The median time to 75% healing was 17.0 and 20.0 days with MEBO and C, respectively (HR = 0.67, 95% CI = 0.41-1.11, P =.11), suggesting similar efficacy between the 2 modalities. Bacterial infection rates were similar between the 2 groups (HR = 1.10, 95% CI = 0.59-2.03, P =.76). MEBO imparted a greater analgesic effect in the first 5 days of therapy and reduced hospital costs by 8%. CONCLUSIONS: MEBO is as effective as conventional management but is not the panacea for all burn wounds. The use of MEBO eases the management of face and neck burns and facilitates early institution of occupational therapy in hand burns. It confers better pain relief such that fewer opiates are used during the first 5 days after burn injury.

摘要:Background: Despite the availability of many topical formulations for scar treatment, an objective evaluation of their effectiveness is lacking. Objective: To assess the physiological, structural, and mechanical parameters of scar healing by a scar ointment (MEBO-Scar). Methods: 45 patients from the Department of Obstetrics at Al Ain Hospital, UAE, with recently acquired Pfannenstiel scars were recruited into this prospective, open-label, controlled study. Scar ointment was applied to a lateral third of the scar; positive control (silicone gel based topical) to the other lateral third; the middle third was left untreated (negative control). The scars were evaluated at 4 time points up to 12 weeks using non-invasive devices and a scar questionnaire. Results: In comparison with silicone gel, there was a significant reduction in trans epithelial water loss at weeks 2, 4, 8, and 12 (p<0.001) and scar area at weeks 4, 8 and 12 (p<0.001) with scar ointment. The skin elasticity at week 4 was significantly increased by scar ointment (p=0.007). By week 12, gross colour mismatch of the scar was described by 9% patients with scar ointment (24% with silicone gel, 42% with no treatment; p<0.001). There was a significant improvement in scar texture with scar ointment by week 12 (p<0.001). Conclusion: Scar ointment has beneficial effects on objective and subjective patient-reported parameters of Pfannenstiel scars.
