基于cGAS-STING通路探讨MEBT/MEBO促进糖尿病创面愈合的作用机制

摘要:目的:本研究将从环鸟苷酸腺苷酸合成酶-干扰素基因刺激因子(Cyclic guanine nucleotide adenine nucleotide synthetase-Stimulator of interferon genes,cGAS-STING)通路入手,明确湿润暴露疗法/湿润烧伤膏(Moist exposed burn therapy/Moist exposed burn ointment,MEBT/MEBO)能否通过调控cGAS-STING信号通路减轻创面组织炎症反应状态,从而加快糖尿病创面修复的进程,为之后的临床治疗和科学研究提供可靠的参考数据。方法:1.通过网络药理学数据库筛选出MEBT/MEBO治疗糖尿病创面的活性成分和关键靶点,并对其进行京都基因和基因组百科全书(Kyoto Encyclopedia of Genes and Genomes,KEGG)和基因本体(Gene Ontology,GO)富集分析和蛋白质相互作用网络(Protein-protein interaction,PPI)分析,对活性成分和关键靶点进行分子对接验证其结合能力。2.动物实验验证:(1)选取48只Wistar雄性大鼠随机分为4组:空白组、MEBO组、贝复新组、对照组,其中空白组构建正常大鼠急性创面模型,MEBO组、贝复新组和对照组构建糖尿病大鼠创面模型。(2)通过HE染色观察各组大鼠造模后第3、7、14天创面愈合情况、组织形态学变化。(3)采用Western Blot、Real-time qPCR检测cGAS-STING通路相关蛋白表达变化差异及mRNA转录水平差异。结果:(1)通过网络药理学方法筛选出77个湿润烧伤膏和糖尿病创面的交集靶点,显示刺槐素、汉黄芩素、槲皮素、吴茱萸次碱等为湿润烧伤膏核心活性成分,对交集靶点进行PPI网络分析筛选出TP53、TNF、HSP90AA1等关键靶点,KEGG富集分析结果显示TNF、PI3K-Akt、AGE-RAGE信号通路在湿润烧伤膏对糖尿病创面的治疗中起到重要作用,GO富集分析结果显示这些交集靶点的生物过程主要涉及炎症反应调节和血管新生过程。我们进一步对核心活性成分和关键靶点进行分子对接,结果显示均具有较强结合活性。(2)创面愈合情况:造模后第3天,各组之间的创面愈合率无明显差异(P>0.05);造模后第7、14天,与对照组相比,空白组、MEBO组和贝复新组的创面愈合...

Nonsurgical Management of Hypertrophic Scars: Evidence-Based Therapies, Standard Practices, and Emerging Methods

摘要:Hypertrophic scars, resulting from alterations in the normal processes of cutaneous wound healing, are characterized by proliferation of dermal tissue with excessive deposition of fibroblast-derived extracellular matrix proteins, especially collagen, over long periods, and by persistent inflammation and fibrosis. Hypertrophic scars are among the most common and frustrating problems after injury. As current aesthetic surgical techniques become more standardized and results more predictable, a fine scar may be the demarcating line between acceptable and unacceptable aesthetic results. However, hypertrophic scars remain notoriously difficult to eradicate because of the high recurrence rates and the incidence of side effects associated with available treatment methods. This review explores the various treatment methods for hypertrophic scarring described in the literature including evidence-based therapies, standard practices, and emerging methods, attempting to distinguish those with clearly proven efficiency from anecdotal reports about therapies of doubtful benefits while trying to differentiate between prophylactic measures and actual treatment methods. Unfortunately, the distinction between hypertrophic scar treatments and keloid treatments is not obvious in most reports, making it difficult to assess the efficacy of hypertrophic scar treatment.

Medical and Cosmeceutical Applications for Enhancement of Partial-Thickness Skin Graft Donor-Site Healing

摘要:Introduction:? After partial-thickness skin graft (PTSG) harvesting, a dermal surface is exposed and an iatrogenic wound is created. Application of various agents has been studied for enhancement of healing process in PTSG donor area. The ideal agent should accelerate healing, provide comfort to the patient, not allow infection, and be easily applicable and cheap. Materials and Methods:? The Cochrane Central Register of Controlled Trials, PubMed, Scopus. and EMBASE databases were screened for words (“skin graft” + “donor”), by using time limits between 1989 and 2018. This study involved a total of 5 systematic reviews and 43 original articles which met the inclusion criteria. Articles were reviewed and parameters such as healing time, pain, infection rate, cosmetic result, and cost were extracted. Results:? Systematic reviews favor moist dressing materials over dry ones considering healing time and patient comfort. Hydrocolloids provide rapid wound healing, and dressing changes do not cause any disruption to the newly regenerated epithelium. Alginates seem to have a longer healing time than hydrocolloid materials. Polyurethane films are characterized by rapid epithelialization and painless monitoring as well as low costs. The epithelialization time of MEBO?, a cosmeceutical product, is reported to be faster than polyurethane films. Local and systemic drug applications warrant further research before routine clinical usage. Conclusion:? Dressing materials are the most commonly used and appropriate group according to the studies performed to date. It is seen that moist dressing materials are more favorable. Although there is not a subgroup with a clear superiority, it can be seen that the hydrofiber and polyurethane film groups are one step ahead in terms of ease of use, patient comfort, epithelialization speed, and scar quality. Polyurethane films have a cost advantage to the hydrofiber group.
