Efficacy of Chinese and Western Medical Techniques in Treating Diabetic Foot Ulcers With Necrotizing Fasciitis of the Lower Leg

摘要:To analyze and evaluate the clinical efficacy of Chinese and Western medical techniques in the treatment of severe diabetic foot ulcers complicated with necrotizing fasciitis of the lower leg and summarize the treatment experience of such patients to identify a new method of limb salvage treatment. A total of 46 patients with severe diabetic foot ulcers and necrotizing fasciitis of the lower leg were treated with such techniques as surgical debridement, bone drilling, open joint fusion, and microskin implantation. Wounds were treated with moisture-exposed burn therapy (a regenerative medical treatment for burns, wounds, and ulcers) and moisture-exposed burn ointment (a traditional Chinese medicine); underlying diseases were also treated effectively. The wound healing time, rate of high amputation, and mortality of these patients were summarized, and the clinical efficacy of such treatments was evaluated. Of the 46 patients enrolled, 38 patients were cured, with a cure rate of 82.61%. The average wound healing time was 130?±?74.37 days. Two patients underwent high amputations, with an amputation rate of 4.35%, and 4 deaths occurred, with a mortality rate of 8.70%. The combination of Chinese and Western medical techniques in the treatment of severe diabetic foot ulcers complicated with necrotizing fasciitis of the lower leg not only effectively saved patients' lives and promoted wound healing but also greatly reduced the rates of high amputation and disability.

摘要:Burn therapy (MEBT)/moist exposed burn ointment (MEBO) is an effective traditional Chinese medicine method to treat diabetic wound, but the mechanism is unclear. Autophagy has been proved to be closely related with wound healing, so MEBO/MEBT is hypothesized to promote diabetic wound healing by regulating autophagy. To explore the mechanism of moist exposed MEBT/MEBO promoting diabetic wound repair. Eighty male Wistar rats were randomly assigned to control (n = 20) and diabetic group induced by intraperitoneal injection of STZ (n = 60), which were further randomly assigned to MEBO, Kangfuxin and model groups (n = 20 each). All rats underwent full-thickness skin resection in the back. Wound healing was dynamically observed and wound tissues were collected at five time points for pathological examination, autophagosome and the expression of PI3K, Akt and mTOR. The healing time in the control group was the shortest, no statistically significant difference was found between the MEBO and the Kangfuxin group (p = 0.76). The morphology of autophagosomes ranged large to small, which was the most obvious in the MEBO group. The mRNA and protein expression of PI3K, Akt and mTOR in each group reached the peak on Day 5, the levels in the MEBO group were the highest (F = 18.43, 19.97, 15.36, p < 0.05). On Day 11, the expression levels in each group began to decline. In this study, we discussed the molecular mechanism of MEBT/MEBO promoting the repair of diabetic ulcer wounds through autophagy and PI3K-Akt-mTOR signalling pathway, which provides a new way for drug design in the future.

摘要:Background MEBO is an analgesic and antibacterial natural agent. Sesame oil extract plays a positive effect in wound healing that may be attributed to its ability to scavenge free radicals and antioxidant properties. Material and methods In this study, MEBO was topically applied to the site of a traumatic oral ulcer to examine its therapeutic effect on pain relief and wound healing. Patients were randomly assigned into two equal groups: group I (MEBO), group II (control), (n?=?15 patients each). Pain was recorded by using the Visual Analogue Scale (VAS), wound size was measured at days 2, 5, and 10. Cytological analysis was also performed using the superficial epithelial cell and keratinization indices. Results There was a statistically significant difference in VAS scores and wound size between MEBO group and control group (p?≤?0.001). Cytological results of MEBO group showed a highly significant improvement (p?≤?0.00001) in re-epithelization and keratinization indices over time. Conclusion Our results showed that the application of MEBO on traumatic ulcer sites has a significant improvement in wound healing and rapid enhancing effect on the re-epithelialization rate.

摘要:Second degree burns treatment plays an important role in healing burns. Many people use Moist Exposed Burn Ointment (MEBO), an oil-based ointment that has been proposed for the treatment of ideal burns, and honey has also been used for the treatment of burns in several clinical trials and provides good results. This study aims to determine the ratio of honey and MEBO effectiveness in epithelialization of second degree burns.Methodsby using the true experiment research method or pure experiment with RAL (Complete Random Design). Results The results showed that giving honey to healing burns gave the best results where there were significant differences (p <0.05) in the formation of collagen fibers and the amount of blood vessel formation compared to MEBO, but in the results of measurements of the extent of burns and epithelial formation did not showed a significant difference (p> 0.05) between honey and MEBO.Conclusion/ConclusionsThe conclusion is that giving honey gives better and more effective results in the formation of collagen fibers and blood vessel formation compared to MEBO. Therefore honey can be an option for the community in dealing with second degree burns, and further research needs to be done on signal transduction (molecular biology) related to wound healing in three phases, namely the inflammatory phase (eg interleukin 2, 6 and others). others), proliferation phase (eg EGF, PDGF, TGF-?) and remodeling phase (eg BMP2).
