The use of the fractional CO2 laser resurfacing in the treatment of photoaging in Asians: Five years long-term results

摘要:Background and Objective The purpose of this clinical paper is to explore the therapeutic effects, healing times, adverse effects, and maintenance periods of using a CO2 fractional laser in the treatment of photoaging in Asian skin. Study Design/Materials and Methods One fractional CO2 laser procedure was performed on the full face in 56 patients with photoaging. Based on the Dover scoring system, we evaluated the degree of skin aging before treatment and at one-month post laser and at five years post laser therapy in 30 of the patients. Statistical analysis was performed by the Wilcoxon's method. Results Thirty of the treated patients have had follow-up for 5 years at this time. The photoaging scores in these thirty patients were significantly changed (P < 0.01) at one month, one year, and five years after the fractional laser treatment, as compared with their baseline. Adverse events seen during this analysis were found to be minimal and not of clinical significance. Conclusions Fractional CO2 laser resurfacing in the treatment of photoaging in Asians is a useful modality with results, for the first time, being shown to have continued efficacy for up to 5 years. 摘要:Burn injuries are considered one of the major sources of trauma that might lead to substantial morbidity and mortality. Severe burns cause multiple organ dysfunction and even sometimes organ failure, leading to debility and subsequent scarring that affects the quality of life and mental health. Consequently, this study is designed to compare the outcomes of two burn treatment modalities in patients suffering from burn injuries who were admitted to the Burn Unit / AL-Hussein Teaching Hospital / Samawa / Al-Muthanna Governorate. A clinical study was established during the period from December 2018 to January 2022 60 patients of both sexes suffered from different burn injuries. All patients are treated with Moist Exposed Burn Ointment -MEBO- or with covered classic methods using silver sulphadiazine cream dressings. Follow-up of treated patients is done to evaluate the treatment outcome. Our study included 44 burned and 16 scalded cases of different age groups, with second and third burn degrees extending on different body surface areas ranging from 30-60% in males and 30-70% in females.  Of 60 patients, 40 females (67%) were between 10 and 40 years old, and 20 males (33%) were between 3 and 36 years old. The mortality percentages were 7 (23.3%) and 2 (6.6 %) cases out of a total of 60 patients for the conventional method and MEBO-treated group, respectively. Sepsis was observed in most dead cases, manifested by foul-smelling wound discharge and necrotic non-viable wound tissues, in addition to pneumonia and bronchitis. Pseudomonas sp, Klebsiella sp, and Proteus sp were isolated from these cases by culture and sensitivity tests.  In conclusion, this study showed that the MEBO cream application in treating burned injuries revealed dramatic curative hopeful results with less liability for infective and septic complications compared with the conventional method, represented by the Sulphadiazine cream application. The authors recommend using treatment in burn units instead of using the routine old conventional method to avoid the expected morbidity and mortality and gain acceptable cosmetic results.

Randomized Control Trial Comparing Moist Exposed Burn Ointment and Honey as Dressings Agents in Patients with Second -Degree Burns

摘要:Background: Many  topical  agents  are  available  for  the treatment  of  partial  thickness  burns.  We examined  Moist  Exposed  Burn  Ointment  (MEBO)  and  honey  as  dressing  agents;  regarding  their natural antibacterial, anti-inflammatory, and wound healing properties. They have also been proven to be superior than silver sulphadiazine in treating burn wound.Methods: A  total  of  34  patients  were  randomly  allocated  into  MEBO  group  and  Indonesian  local honey group. The dressings are changed  daily, with corresponding interventions applied. The depth and extent of burn wounds were assessed upon patient’s admission and once a week for two weeks. Pain  level was  evaluated  using  the Numeric  Rating  Scale  (NRS).  The  wound  swabs  were  cultured weekly to obtain microorganisms profile. The cost-per-day of each group were counted.Result: Acute  partial  thickness  burn  in  MEBO  group  showed  faster  healing  compared  to  honey group. NRS  scores for  pain were also lower in MEBO group. MEBO was found  to be more effective in decreasing bacterial colonization. Cost of  treating wound  using MEBO until complete healing was also lesser than local honey.Conclusions: As  topical agent  to  treat  partial  thickness burns,  MEBO  is  superior  compared  to  local honey due to faster healing, better pain relief, fewer bacterial colonization and the more cost-effective usage.

摘要:Purpose: To investigate the bioactive components and mechanism of action of moist exposed burn ointment (MEBO) for treatment of diabetic foot ulcers using network pharmacology (NP) and molecular docking technology Methods: Through pharmacology database of traditional Chinese medicine (TCM) systems (TCMSP), analysis platform and symptom mapping (SymMap) database, the bioactive components of MEBO were screened for and protein binding to bioactive components was predicted. Proteins related to Diabetic foot ulcer (DFU) were collected from GeneCards, OMIM, PharmGkb and TTD disease databases. With R language software, the binding proteins of bioactive components of MEBO intersected with the proteins related to occurrence of DFU. Proteins of DFU linked to treatment with MEBO were subjected to analysis using gene ontology (GO) and KEGG with R language software. AutoDock and PyMOL software were employed to dock active components of MEBO and major proteins of DFU. Targeted binding potential of bioactive compounds in MEBO to core proteins of DFU was analyzed. Results: One hundred and five (105) bioactive ingredients and 246 likely therapeutic proteins were obtained. Proteins were mainly involved in biological processes in wound healing, oxidative stress, and lipopolysaccharide. The enriched signaling pathway focused on lipid metabolism and the process of atherosclerosis which involved PI3K and Akt, advanced glycation end-products (AGE)-receptor (RAGE) and mitogen-activated protein kinase (MAPK). The absolute values of these proteins were screened out with a docking score greater than 5 kcal/mol. Conclusion: The biological effects of MEBO are related to multiple proteins and multiple signaling pathways. Therefore, healing effect of MEBO on DFU occurs via multiple pathways. The biological characteristics of MEBO need to be fully elucidated in further studies.
