Time-dependent morphological and biochemical changes following cutaneous thermal burn injury and their modulation by copper nicotinate complex: an animal model

摘要:BACKGROUND: Thermal tissue injury is partly mediated by reactive oxygen metabolites. Oxygen free radicals are contributory to local tissue damage following thermal injury and accordingly an interventional therapy using antioxidants may be beneficial. Copper nicotinate complex can scavenge reactive oxygen species (i.e., has antioxidant activity). OBJECTIVES: To examine time-related morphological and biochemical changes following skin thermal injury and their modulation by copper nicotinate complex. MATERIALS AND METHODS: An animal model composed of 80 albino rats was established. Ten rats (nonburn group) served as a control group. Seventy rats (burn group) were anesthetized, given a 10% total body surface area, full-thickness burn. Ten rats (from the postburn group) were sacrificed after 24 h (without treatment, i.e., untreated-burn group). The remaining rats were divided into three subgroups (20 rats, each) and were treated topically either with soft paraffin, moist exposed burn ointment (MEBO, a standard therapeutic treatment for burns), or copper nicotinate complex. Five animals from each subgroup were sacrificed every week over a period of 4 weeks. The morphological and biochemical changes were evaluated and compared among the different groups. RESULTS: High levels of the plasma and skin nitiric oxide (marker of oxidative stress) were observed in the untreated-burn group. These levels were significantly low following the application of copper nicotinate complex. Low levels of plasma and skin superoxide dismutase (marker of oxidative stress) and plasma ceruloplasmin were observed in the untreated-burn group. These levels were significantly high following copper nicotinate complex treatment. The total and differential leukocyte counts were low following the onset of the thermal injury. They gradually returned to normal levels over a 4-week period following the application of MEBO or copper nicotinate complex. Compared to untreated-burn group, postburn-healing changes (resolution of the inflammatory reaction, reepithelization of the epidermis, angiogenesis, deposition of collagen fibers, and recovery of the subcellualr organelles) were significantly accelerated following the application of either MEBO or copper nicotinate complex. CONCLUSIONS: Application of copper nicotinate complex was associated with improved healing of the thermal burns of the skin. The underlying molecular changes underlying these effects await further investigations.

The Use of MEBO Scar Ointment in the Treatment and Prevention of Post-Operative Wound Scars

摘要:Background: Scar formation is a natural part of the wound healing process. The aim of this study was to observe the therapeutic effect of MEBO Scar ointment in the prevention and treatment of incisional scars. Methods: MEBO Scar ointment was applied 3 times daily after surgical stitches were removed in the test group. Warm saline was used in the control group. Clinical efficacy was based on scar quality assessment measuring – itching relief, skin texture, dystrophy and color. The wound was assessed at pre-determined intervals over 120 days post-surgery for therapeutic effectiveness of the ointment. Results: A total of 90 patients, male and female, undergoing elective surgery between February and December, 2013, with mean age of 30 years were included in this study. At 120 days, MEBO Scar was found to be highly effective (scar disappearance, no itching with soft scar, skin colour close to normal tissue) in majority patients (75%); and the overall scar quality was found to be significantly better (p<0.001) for all parameters. Conclusion: When initiated early, standard use of MEBO Scar Ointment for adequate period of time sufficiently softens the scars to prevent proliferation and appearance disturbances, resulting in a cosmetically acceptable scar. Therefore, it may be recommended for the prevention and treatment of scars postsurgery.

The signaling pathways of traditional Chinese medicine in promoting diabetic wound healing

摘要:Ethnopharmacological relevance The diabetic wound is one of the common chronic complications of diabetes, which seriously affects patients’ quality of life and even causes disability and death. Traditional Chinese medicine (TCM) is a unique and precious resource in China, which has a good curative effect and safety. At present, it has been found that Chinese herbal compounds and effective active ingredients can effectively promote diabetic wound healing, and its mechanism needs to be further studied. Signaling pathways are involved in the pathogenesis and progression of diabetic wounds, which is one of the main targets for the pathologic mechanism of diabetic wounds and the pharmacological research of therapeutic drugs. Aim of the review This study has been carried out to reveal the classical signaling pathways and potential targets by the action of TCM on diabetic wound healing and provides evidence for its clinical efficacy. Materials and methods “diabetic wound”, “diabetic foot ulcer”, “traditional Chinese medicine”, “natural plant” and “medicinal plant”, were selected as the main keywords, and various online search engines, such as PubMed, Web of Science, CNKI and other publication resources, were used for searching literature. Results The results showed that TCM could regulate the signaling pathways to promote diabetic wound healing, such as Wnt, Nrf2/ARE, MAPK, PI3K/Akt, NF-κB, Notch, TGF-β/Smad, HIF-1α/VEGF, which maintaining inflammatory interaction balance, inhibiting oxidative stress and regulating abnormal glucose metabolism. Conclusion The effect of TCM on diabetic wound healing was reflected in multiple levels and multiple pathways. It is envisaged to carry out further research from precision-targeted therapy, provide ideas for screening the core target of TCM in treating diabetic wounds and create modern innovative drugs based on this target.
