Wound Management of Multi-Site Pressure Ulcer at Different Stages in Elderly Patients

摘要:Objective The present study aims to explore the individualized treatment options for multisite pressure ulcer (PU) at various stages in elderly patients with multiple medical conditions. Methods Stages 1 and 2 PU at 146 sites were treated with closed negative pressure suction combined with continuous micro-oxygen perfusion and the local application of foam dressings, silver ion dressings, and moist burn cream. Stages 3 and 4 PU in the sacrococcygeal region were treated with skin or myocutaneous flap transplantation. Results Stages 1 and 2 PU healed after treatment with closed negative pressure suction combined with continuous micro-oxygen perfusion and dressing changes. One case died during hospitalization due to an illness. Skin or myocutaneous flap repair was conducted in 34 cases of stage 3 or 4 PU in the sacrococcygeal area. Of these cases, 28 achieved primary healing, and 6 required two or three surgeries, 5 of which received micro-skin implantation. In addition, 10 small deep PU at other sites were repaired by direct excision and suturing or local flap repair. Seven cases were transferred to other departments or hospitals due to concomitant diseases or were discharged automatically without surgical treatment. Conclusion Home care for geriatric patients is difficult. PU often occur at multiple sites because of the duration of various pressures, and different sites may demonstrate different stages because of varying degrees of pressure. When actively treating stages 3 and 4 PU, the trauma management of stages 1 and 2 PU should not be neglected.

Wound healing in immunocompromised dogs: A comparison between the healing effects of moist exposed burn ointment and honey

摘要:Background and Aim: Natural topical products have been used to enhance wound healing, especially in immunocompromised animals. The aims of this study were to evaluate and to compare the effects of moist exposed burn ointment (MEBO) and honey on the healing of full-thickness skin wounds in immunocompromised dogs. Materials and Methods: The study was conducted using 12 adults, apparently healthy mongrel dogs. Immunosuppression was induced in six dogs by oral administration of prednisone (2 mg/kg) and azathioprine (2 mg/kg), once a day for 21 days. On each dog, a total of 9, 1.5 cm in diameter full-thickness skin circular wounds were created aseptically in the thoracolumbar area under general anesthesia using customized skin punch biopsy kit. In a random fashion, three wounds in each dog were treatment using MEBO (contains b-sitosterol, baicalin, and berberine as active ingredients in a base of beeswax and sesame oil), honey or no treatment (control), once per day for 21 days. Wounds were grossly evaluated once a day for signs of inflammation or infection. In addition, biopsy specimens and digital imaging data of each wound were obtained on days 7, 14, and 21 for histopathological evaluation of the healing process. Results: Wounds in immunocompromised dogs appeared to heal significantly in a slower fashion than in non-immunocompromised counterparts. Digital analysis data showed that MEBO-treated wounds expressed better epithelialization area, faster contraction, and smaller wound area percentage when compared with honey-treated wounds. Histopathological analysis showed significantly higher angiogenesis scores in MEBO-treated wounds when compared with other treatments. Conclusion: Results of this study showed that MEBO resulted in significant enhancement of wound healing in both healthy and immunocompromised dogs. However, when compared to honey, the wound healing effect of MEBO was superior to that of honey.

The Use of MEBO Scar Ointment in the Treatment and Prevention of Post-Operative Wound Scars

摘要:Background: Scar formation is a natural part of the wound healing process. The aim of this study was to observe the therapeutic effect of MEBO Scar ointment in the prevention and treatment of incisional scars. Methods: MEBO Scar ointment was applied 3 times daily after surgical stitches were removed in the test group. Warm saline was used in the control group. Clinical efficacy was based on scar quality assessment measuring – itching relief, skin texture, dystrophy and color. The wound was assessed at pre-determined intervals over 120 days post-surgery for therapeutic effectiveness of the ointment. Results: A total of 90 patients, male and female, undergoing elective surgery between February and December, 2013, with mean age of 30 years were included in this study. At 120 days, MEBO Scar was found to be highly effective (scar disappearance, no itching with soft scar, skin colour close to normal tissue) in majority patients (75%); and the overall scar quality was found to be significantly better (p<0.001) for all parameters. Conclusion: When initiated early, standard use of MEBO Scar Ointment for adequate period of time sufficiently softens the scars to prevent proliferation and appearance disturbances, resulting in a cosmetically acceptable scar. Therefore, it may be recommended for the prevention and treatment of scars postsurgery.

The signaling pathways of traditional Chinese medicine in promoting diabetic wound healing

摘要:Ethnopharmacological relevance The diabetic wound is one of the common chronic complications of diabetes, which seriously affects patients’ quality of life and even causes disability and death. Traditional Chinese medicine (TCM) is a unique and precious resource in China, which has a good curative effect and safety. At present, it has been found that Chinese herbal compounds and effective active ingredients can effectively promote diabetic wound healing, and its mechanism needs to be further studied. Signaling pathways are involved in the pathogenesis and progression of diabetic wounds, which is one of the main targets for the pathologic mechanism of diabetic wounds and the pharmacological research of therapeutic drugs. Aim of the review This study has been carried out to reveal the classical signaling pathways and potential targets by the action of TCM on diabetic wound healing and provides evidence for its clinical efficacy. Materials and methods “diabetic wound”, “diabetic foot ulcer”, “traditional Chinese medicine”, “natural plant” and “medicinal plant”, were selected as the main keywords, and various online search engines, such as PubMed, Web of Science, CNKI and other publication resources, were used for searching literature. Results The results showed that TCM could regulate the signaling pathways to promote diabetic wound healing, such as Wnt, Nrf2/ARE, MAPK, PI3K/Akt, NF-κB, Notch, TGF-β/Smad, HIF-1α/VEGF, which maintaining inflammatory interaction balance, inhibiting oxidative stress and regulating abnormal glucose metabolism. Conclusion The effect of TCM on diabetic wound healing was reflected in multiple levels and multiple pathways. It is envisaged to carry out further research from precision-targeted therapy, provide ideas for screening the core target of TCM in treating diabetic wounds and create modern innovative drugs based on this target.
